HIPAA authorization form – private health information

The HIPAA authorization form is required when private healthcare information is provided to third party individuals or entities not involved directly with the patient’s care or billing for that care. Without the completion of such a form, HIPAA requires that private health information remain confidential.

The HIPAA authorization form is to outline the specific party that the private health information should be shared with, the exact nature of the information that is going to be shared, and the length of time that the information will be available to that party.

Once the patient understands the authorization form, he or she signs their name to complete the authorization. This authorization form is required to be kept on file as private medical information.
